Send each guest a link to complete their details, the diver medical questionnaire and the liability release on their phone before they arrive. DiveHQ uses the current WRSTC/UHMS Diver Medical Participant Questionnaire with its follow-up questions, so a guest is referred to a physician only when their answers require it.

Medical answers that need a doctor are flagged until the guest is cleared.
What it does
The current WRSTC/UHMS medical questionnaire with its follow-up questions, so minor history such as childhood ear grommets does not automatically require a doctor's clearance.
Each signature is stored with the signer's typed name, the time, the IP address and a SHA-256 hash of the signed text.
Staff can complete forms with a guest on a tablet at the counter, and the record shows it was done in person.
Reminders sent seven days before arrival if forms are incomplete, and the day before arrival with what to bring.
Form status for every guest: not sent, sent, partly complete, complete, needs clearance or cleared.
Choose whether a missing release blocks check-in or only shows a warning.
Check in a walk-in guest by creating the guest, the booking and the check-in on one screen.
